Forward Momentum is not currently accepting insurance. Lori can provide a superbill for possible out-of-network reimbursement; clients should verify their own coverage.

Please provide 24 hours notice of cancellation to avoid being charged in full for the scheduled session. Major credit/debit cards are accepted.
