Frequently Asked Questions

Q1:Which insurance plans does the practice accept?

The site says the practice gladly accepts commercial insurances and Medicaid HMOs. Its intake form for in-network patients names Affinity, FDNY, Medicare, Metroplus, No-Fault (motor vehicle accident) and Workers' Compensation. If you are unsure whether your plan is accepted, the site asks you to call the office.

Q2:Will the office check my benefits before I come in?

Yes. The site says that, as a courtesy, the office will verify your health plan benefits before your evaluation. It adds that your plan may require a co-pay or a percentage of the bill.

Q3:What should I bring to my first visit?

The site says to bring the physical therapy referral from your doctor and your payment information. Bring your insurance card if insurance is covering the cost. If you are covered by Workers' Compensation, bring your claim number and your case manager's contact information, and bring auto insurance or attorney lien details if they apply.

Q4:What happens at the first visit?

You fill out paperwork, give the office your prescription for physical therapy, and the office copies your insurance card. The therapist then reviews your medical history, complaints, pain and goals and performs an evaluation, which may include range of motion, muscle and neurological screening and posture assessment. A plan is developed with your input, and with your doctor's.

Q5:How long does each treatment last?

The site says a treatment can last between 45 and 60 minutes, depending on your condition.

Q6:How often will I come, and for how long?

The site says patients usually come 2 to 3 times a week for 1 to 2 months, although the length of treatment may differ for each patient.

Q7:What is the cancellation policy?

The site asks you to notify the office within 12 hours if you cannot attend so the schedule can be adjusted. Failing to follow the 12-hour cancellation policy results in a $25.00 fee charged to the patient.
